Federal Action on Kratom-Related Compounds May Leave Consumers Struggling to Identify Product Differences, National Detox Provider Warns

Three synthetic kratom compounds became Schedule I substances on August 26. A separate federal rule covering concentrated 7-OH has not been issued. Elite Home Detox says the packaging looks the same either way, and physical dependency does not follow the legal line.

United States, September 16, 2026 -- Elite Home Detox, a national provider of in-home, medically supervised detox care operating in more than 50 cities nationwide, is warning that the federal government's split approach to kratom-derived products has created a gap that consumers and clinicians are not equipped to navigate.

On August 26, 2026, a Drug Enforcement Administration temporary scheduling order took effect placing three synthetic compounds associated with kratom, mitragynine pseudoindoxyl, MGM-15, and MGM-16, into Schedule I of the Controlled Substances Act. The order runs through August 26, 2028 and took effect the day it was published. In animal studies cited in the federal record, MGM-15 was found to be roughly 50 times more potent than morphine and MGM-16 roughly 240 times.

A separate federal action covering 7-hydroxymitragynine, known as 7-OH, above a specified concentration threshold has not yet produced a scheduling order. Federal officials reopened public comment on the proposed threshold in late August. Neither action covers natural kratom leaf that has not been chemically enhanced.

For someone standing at a convenience store counter, none of that is visible on the label.

"Consumers encounter very different products marketed under the same broad term," said Fred Bass, CEO of Elite Home Detox. "Some kratom-related compounds became Schedule I substances in August. Concentrated 7-OH is the subject of a separate DEA process, while natural kratom leaf falls outside these specific scheduling actions. Labels may not make those distinctions clear, so clinicians need to ask specifically what product a person is using."

Dependency does not follow the legal line

Elite Home Detox has treated kratom dependency for years alongside alcohol, benzodiazepines, opiates, and cocaine, through medically supervised in-home programs typically lasting five to eight days. The company says the pattern it sees is consistent, and it is not the pattern most people associate with addiction treatment.

Clients who develop dependency on concentrated kratom products generally did not obtain them through an illicit market. They bought them at retail. Many do not describe themselves as having a substance use problem, and do not consider themselves candidates for a treatment facility. When supply is interrupted, whether by a scheduling order, a state restriction, or a store simply pulling inventory, the physical dependency remains.

Withdrawal from concentrated kratom-derived compounds presents similarly to opioid withdrawal and can include sweating, chills, muscle aches, nausea, gastrointestinal distress, and insomnia.

"The clinical risk here is not the person who knows they need help. It is the person who does not think this applies to them," said Alisha Bass, CADC I, Director of Operations at Elite Home Detox. "If someone tells you they take a supplement they picked up at a gas station, that answer needs a follow-up question about the product, the dose, and how long they have been taking it. A patient may be describing an opioid dependency without ever using the word. If we are not asking specifically, we are not seeing it."

What Elite Home Detox recommends

The company is encouraging clinicians, facility partners, and referral sources to screen specifically for kratom and 7-OH product use during intake, including product type, concentration, dose, and route of use, rather than relying on general substance use questions.

For individuals currently using concentrated kratom-derived products on a daily basis, Elite Home Detox recommends speaking with a clinician about a planned taper rather than stopping abruptly, particularly given the possibility of further federal action.

"Nobody should be finding out they are physically dependent on something because it disappeared from the shelf," Bass said. "There is a window right now to handle this as a medical decision instead of an emergency. That window closes the moment the next order publishes."
